Turning Around a 46-Bed Care Home in Surrey

The Challenge
A 46-bed residential and nursing care home in Surrey was facing a serious decline in service quality. The home supported individuals with physical disabilities and complex health needs, but ongoing issues with staff morale, poor communication, and cultural tensions had caused significant disruption to daily operations. Regulatory requirements were being routinely missed, and a lack of training compounded existing risks.
The situation escalated when the local authority issued a Notice of Proposal (NOP), putting the provider on the brink of losing its operating licence. With regulatory pressure mounting, the provider approached Fulcrum Care to provide interim leadership and deliver urgent, on-the-ground change.
Our Approach
Fulcrum Care deployed a consultant for an initial three-month engagement, with a clear objective: to deliver visible improvements within the first six weeks. From the outset, the consultant worked within the home’s existing systems, aligning all actions with local authority expectations.
The early focus was on stabilising the workforce. Structured team-building sessions and open forums helped rebuild trust among staff. Clear expectations around communication and professional conduct were re-established, creating a more respectful and collaborative work environment.
At the same time, Fulcrum conducted a full compliance review. Key actions, including mandatory training and regulatory updates, were brought back on track through hands-on oversight and clearer accountability. Cultural competence training was also introduced to help unify a previously fragmented team.
Throughout, Fulcrum worked in close partnership with the local authority, ensuring transparency and alignment at every stage of the improvement plan.
The Outcome
Within four weeks, the local authority carried out an unannounced inspection. Inspectors reported marked improvements in structure, communication, and overall service delivery. This progress prompted the provider to extend the consultancy engagement by a further six months, recognising the need for continued support to embed long-term change.
By the end of the project, the care home had been removed from the provider of concern list. Staff were more engaged, compliance processes had stabilised, and the local authority praised Fulcrum Care’s structured approach and visible impact.
